Students enrolled in the Massage Therapy Program must email their medical requirements to [email protected] or [email protected] within 30 days prior to the start of every academic semester they are enrolled in the program.

The following are the massage therapy program requirements:

  1. History and Physical Examination (due yearly)

  2. Urinalysis (due yearly)

    • Routine and microscopy (lab report required) or chemical dipstick (Practitioner note required)

  3. QuantiFERON-TB Gold (lab report required - due yearly)

    • Chest x-ray report required only for positive QuantiFERON-TB Gold

  4. Tdap (Tetanus, diphtheria, acellular pertussis) vaccination (every 10 years)

  5. Influenza vaccination (due yearly every Fall) (Lot # and Expiration Date)

    • Influenza vaccine date received, lot number and expiration date are required annually. This vaccine comes out every year in August and is good through June of the following year.

  6. Hepatitis B vaccination series

  7. Hepatitis B surface antibody (HBsAb) titer (lab report required)

  8. Hepatitis B surface antigen (HBsAg) titer (lab report required)

  9. Complete blood count (CBC) with differential (lab report required)

  10. MMR (measles, mumps, rubella) vaccination series

  11. Rubeola (measles) IgG titer (lab report required)

  12. Mumps IgG titer (lab report required)

  13. Rubella (German measles) IgG titer (lab report required)

  14. Varicella IgG titer (lab report required)

  15. Varicella vaccination series

  16. Proof of COVID vaccination date, name of vaccine received, lot number, expiration date.

    COVID vaccination is required as per our clinical agencies. Covid vaccine requirements will be in accordance with policies of the clinical facilities. There will be no exception to this rule. 1COVID vaccine is required preferably the bivalent one. Send a copy of your COVID vaccination card for verification (as a PDF) in with the rest of your medical documentation to [email protected] and give your professor a copy.


It is the student's responsibility to inquire about their medical requirements 60 days prior to the start of the academic semester. Therefore, students are required to email [email protected] or [email protected] the medical documentation at least 30 days prior to the start of the academic semester.

It is important that all massage therapy medical requirements be completed before they are submitted to the Office of Health Services.

Massage Therapy Students must make two (2) copies of all medical records. Health Services will not make copies for you.

At these places medical providers are not allowed to turn you away if you cannot pay for care or because of your immigration status. They offer a sliding fee scale based on family size and income (depending on your income, you may pay nothing).
